C-X-C chemokine receptor type 6 (CXCR6), also known as or CD186, is a seven-transmembrane G protein-coupled receptor primarily expressed on activated T cells, NK/NKT cells, and selected tissue-resident memory T-cell populations. Its only known ligand, CXCL16, exists in both membrane-bound and soluble forms. Binding of CXCL16 to CXCR6 regulates immune-cell chemotaxis, adhesion, tissue retention, and survival, while activating downstream signaling pathways such as PI3K/AKT and MAPK.

The CXCR6/CXCL16 axis plays a context-dependent role in immunity and disease. Within the tumor microenvironment, CXCR6 can promote the recruitment and retention of cytotoxic T cells in tumor tissues, thereby supporting antitumor immune responses and potentially improving responses to immune checkpoint inhibitors. However, CXCR6 expression on certain tumor cells may also contribute to proliferation, invasion, metastasis, and treatment resistance. CXCR6-related research has been reported in hepatocellular carcinoma, prostate cancer, melanoma, breast cancer, gastric cancer, and other malignancies, as well as in multiple sclerosis, rheumatoid arthritis, inflammatory bowel disease, nonalcoholic steatohepatitis, graft-versus-host disease, infectious diseases, and cardiovascular inflammation.

No direct CXCR6-targeted therapy has yet been approved. Current drug-development strategies mainly include monoclonal antibodies designed to block or deplete CXCR6-positive pathogenic T cells, small-molecule CXCR6 antagonists, and engineered cell therapies that enhance CXCR6 expression to improve the tumor homing of CAR-T cells, TCR-T cells, or tumor-infiltrating lymphocytes. Most CXCR6-targeted programs remain at the preclinical or early translational stage.
