Epidermal Growth Factor Receptor (EGFR), also known as ERBB1 or HER1, is a single-pass transmembrane receptor tyrosine kinase and a central member of the ERBB receptor family. Its structure comprises an extracellular ligand-binding region with four subdomains, a transmembrane helix, an intracellular kinase domain and a regulatory C-terminal tail. Ligand binding promotes receptor dimerization and phosphorylation, activating major signaling networks such as RAS–RAF–MEK–ERK, PI3K–AKT–mTOR, PLCγ–PKC and JAK–STAT. These pathways regulate cell proliferation, survival, differentiation, migration and tissue repair. Activating mutations, gene amplification, receptor overexpression and other forms of pathway dysregulation are found across multiple solid tumors, making EGFR both an established biomarker and a major therapeutic target. Its broad expression and complex resistance biology also make assay design, epitope selection and model choice especially important during drug discovery.

EGFR drug development is mature but continues to evolve. Small-molecule tyrosine kinase inhibitors and extracellular-domain monoclonal antibodies have already reached regulatory approval in multiple oncology settings. Current research focuses on expanding mutation coverage, improving central nervous system penetration, reducing wild-type EGFR-related toxicity and overcoming acquired resistance. Mutation-selective and allosteric inhibitors range from approved products to late-stage clinical programs, while combination regimens with chemotherapy, immunotherapy or other targeted agents are being evaluated in mid- to late-stage trials. Antibody-drug conjugates and bispecific or multispecific antibodies are mainly in early- to mid-stage clinical development, aiming to improve payload delivery, increase tumor selectivity or block parallel resistance pathways. T-cell engagers, cell therapies, targeted degraders and other emerging modalities remain largely preclinical or early clinical. Across these approaches, biomarker-based patient selection, resistance profiling and longitudinal molecular monitoring are increasingly important for matching therapies to tumor biology.
